This is a supplemental Registration Form which is to be completed upon museum request by guardians who have registered more than one child per order for the Children’s Museum’s upcoming 2024 Summer Day Camps.
PLEASE NOTE: ONE REGISTRATION FORM MUST BE SUBMITTED PER CHILD.
